因此這頁，作為參考，試著準備了幾個有代表性的例子，請活用。
再者，名字和帳號號碼，金額和日期等(出現的部分)請留意藍色字的資料請自己另寫並必要變更。同時，發送郵件的時候，請必定從登記了的電郵地址寄到娛樂場。從其他的地址送的話，也有可能不被確認
